Are School Obesity Programs Causing Disordered Eating Habits in Children?

As the fight to combat childhood obesity enters full swing, many parents are left wondering if school-related programs promote questionable eating behaviors and physical activities in their children. Science Daily explores this issue, and, citing a recent poll, finds that even the most well-intentioned efforts to decrease childhood obesity could create a most unintentioned outcome.

School Obesity Programs May Promote Worrisome Eating Behaviors and Physical Activities in Kids

In a new poll, 30% of parents report at least one worrisome behavior in their children that could be associated withthe development of eating disorders.

A new report from the C.S. Mott Children's Hospital National Poll on Children's Health examines the possible association between school-based childhood obesity prevention programs and an increase in eating disorders among young children and adolescents.

The poll asked parents about obesity prevention programs in their children's schools and about food-related behaviors and activity that may be worrisome.

Overall, 82 percent of parents of children age 6-14 report at least one school-based childhood obesity intervention program taking place in their child's school. Among these programs are nutrition education, limits on sweets or "junk food" in the classroom, height and weight measurements, and incentives for physical activity.

Additionally, 7 percent of parents report that their children have been made to feel bad at school about what or how much they were eating.

This same group of parents was also asked about their children's eating behaviors. Thirty percent of parents of 6-14 year-olds report least one behavior in their children that could be associated with the development of an eating disorder. These behaviors include inappropriate dieting, excessive worry about fat in foods, being preoccupied with food content or labels, refusing family meals, and having too much physical activity.

"The issue of childhood obesity is a serious problem. In order to intervene in what seems like an epidemic of childhood obesity, everyone needs to be involved," says David Rosen, M.D., M.P.H., Clinical Professor of Pediatrics, Internal Medicine, and Psychiatry at the University of Michigan Medical School and Chief of Teenage and Young Adult Medicine in the Department of Pediatrics.

However, Rosen says, "When obesity interventions are put in place without understanding how they work and what the risks are, there can be unintended consequences. Well-intentioned efforts can go awry when children misinterpret the information they're given.

"Many of these behaviors are often dismissed as a phase," says Rosen, "But given what we know about the association of these behaviors with the development of eating disorders and knowing that eating disorders are increasing in prevalence, they should be taken very seriously."

Parents that report incentive programs at their children's school to increase physical activity are more likely to say their children are "too physically active" (11%) compared with parents who do not report incentives for physical activity at their child's school (4%). Otherwise, the poll did not find an association between school-based obesity prevention programs and other worrisome eating behaviors among children.

The fact that 30% of parents report at least one worrisome eating behavior in their children is concerning.

"It's much better and safer for parents to respond to worrisome eating behaviors early -- even if there turns out to be no problem -- than to wait until there is obviously a big problem,' Rosen says. "It is much easier to prevent an eating disorder than it is to treat an eating disorder."

Rosen offers these suggestions for parents:

1. Be attentive to your children's eating habits. If you see behaviors that are worrisome to you, talk to your children about them. If the behaviors escalate, involve your child's doctor.

2. Find out what your children's schools are doing to prevent childhood obesity. Be involved and engaged in that process.

3. Ask your children if they're being teased at school about their food choices or their weight. If they are, go to the school

What Dialectical Behavior Therapy (DBT) Assumes About You

That’s right, we don’t even know you and already we are jumping to conclusions. But before you dismiss this post, please read on. Individuals with eating disorders have often experienced many different treatments across different levels of care, which may lead to feelings of hopelessness and self-blame. Thoughts such as “I’m never going to get better” or “something must be wrong with me” are common. Sometimes even those that are in a position to be helpful and supportive, like family, friends, or even treatment providers, give in to the notion that the individual “isn’t trying,” “doesn’t want to get better,” or “is destined to be sick for the rest of his or her life.” Not only are these ideas incorrect, but they are harmful and prevent those who are suffering from getting the help they need and deserve.

In Dialectical Behavior Therapy (DBT), we explicitly commit to the following assumptions about our clients, their families, other treatment providers and ourselves. Why? It works. It’s effective in helping us achieve our goals. It allows us to come from a place of acceptance and compassion and to maintain hope and certainty that recovery is possible for everyone. It is a commitment to provide the best treatment possible. So what are these assumptions we speak so fondly of?

You are doing the best that you can. In any given moment or situation, no matter what the behavior or outcome, DBT firmly believes you are doing your very best. If you could do better, you would. Yesterday’s binge was not an attempt to cause difficulties or make things worse; it was an attempt at survival and the best you could do in that moment.

You want to improve. While imaging life without your eating disorder may be scary, DBT believes that all those suffering truly do desire to recover. No one chooses to have an eating disorder. No one chooses to experience emotional pain, tormenting thoughts and self-hatred on a regular basis. There are likely many things that contribute to lack of improvement, such as environmental factors or faulty beliefs, but lack of intent or desire is not one of them.

You must learn new behaviors both in therapy and in the context of your day-to-day life. Wanting to recover is not enough. You must actively participate in learning new behaviors and be willing to try new things, even if you’re skeptical or fearful of failure. So yes, you must give self-soothing skills, mindfulness and distracting activities a chance. And not just in the therapy room, but in your regular life where it really matters. Don’t worry, you’re not on your own. DBT skills group is specifically designed to teach you new skills to try!

You cannot fail in DBT. If a cancer patient received chemotherapy and the cancer continued to spread, we would not tell the patient they failed chemotherapy. We would say the chemotherapy failed them. The same goes for DBT. If you participate in treatment and do not improve, the treatment failed you. Maybe it was not implemented effectively or maybe DBT was not the right treatment for you in the first place or at that particular time in your life. Bottom line, you did not fail.

You may not have caused all of your problems, but you have to solve them anyway. While it may seem unfair, it is reality. Assigning blame to yourself or others, or waiting for others to fix your problems for you is not effective. It does not bring you any closer to building the life that you want. It is effective to identify these problems, solve those that can be solved, and learn to accept those realities that cannot be changed.

You need to do better, try harder and be more motivated to change. While DBT firmly believes you are doing your very best, you must also keep trying harder. Life and recovery are all about continuing to learn and grow, which takes an extraordinary amount of effort and hard work. This means continuing to experiment with new ways of coping, allowing others to help you, facing your fears head on, and being gentle with yourself in the process. To commit to this journey, you must have something to work towards that is meaningful for you personally, such as graduating college, building relationships, bringing joy back to your life, or being present for your family.

Your life is unbearable as it is currently being lived. Life with an eating disorder is miserable. The physical, emotional, cognitive, interpersonal and spiritual suffering can be so unbearable that self-destructive behaviors may be the only way you’ve survived. While this is completely understandable, it is not okay. Settling is not okay. You deserve to build a life you truly want to live… and you can.

10 tips to enjoy a recovery-focused holiday

It’s that time of year again. Pitfalls to your eating disorder lurk around every corner, whether it's a plethora of holiday parties, cookies at the office, eggnog at the neighbors or grandma’s homemade fudge. Below are some tips to have a happy and healthy holiday season:

Eat regularly throughout the day. Avoid “saving” or “banking” your calories in order to indulge at holiday parties. Arriving famished to an event pretty much guarantees an evening of poor choices.
Practice mindful eating habits. Eat slowly and take breaks. Remember, it takes 20 minutes for your stomach to let your brain know it’s full.
Get enough sleep. Lack of sleep increases cravings for sugary “fixes”. In addition, adequate rest enables you to manage holiday stressors and avoid emotional eating.
Use portion control. Portioning your food can be a helpful strategy when faced with an abundance of foods.
Control your environment. You are more likely to overeat or feel overwhelmed if you are socializing near the appetizer and treats. Involve a supportive person to help you manage stress at holiday gatherings.
Allow yourself to say “no thank you”. It is perfectly reasonable to politely refuse food. Don’t let “food pushers” sabotage your efforts to make positive choices during the holiday season.
Bring a dish you feel comfortable eating to your holiday parties. Your family and friends will likely welcome another option.
Make an effort to maintain exercise routines. Regular exercise will help manage holiday stress and it may promote positive body image.
Limit your alcohol intake. Alcohol stimulates your appetite and decreases your ability to practice mindful eating habits.
Divert your attention. Focus on catching up with family and friends, rather than the food. Holiday activities, games, and dancing are nice alternatives to focusing on food.

How “the talking cure” helped Diane Keaton overcome her struggle with bulimia

Legendary actress Diane Keaton has been getting a lot of press recently after she opened up about her struggle with bulimia in her 20’s. In her new memoir, Keaton admits that when she started out in show business her poor self-image drove her to bulimia. Her weight loss started a five-year battle with this serious eating disorder.

Keaton sat down with Ellen Degeneres last week and opened up about her struggle with bulimia. After undergoing psychotherapy – what she called the 'talking cure' – Keaton said she one day realized that she did not want to binge any more. When asked to elaborate on how this therapy helped her, she explained, “Because I talked. I spoke it out. I said my thoughts and feelings. And I feel like, once you do that, you own it as opposed to, if you don`t talk about it, it becomes very abstract. To keep secrets doesn’t help you at all,” she said. “I think I'm a sister to all the rest of the women -- and I'm sure men as well -- who have had some kind of eating disorder, and I'm a part of the team.”

The Role of Trauma-Based Therapies for the Treatment of Eating Disorders: What Does Safe and Ethical Treatment Consist Of?

Recent allegations of a treatment center therapist implanting false memories during clinical hypnosis has many people asking, “When it comes to clinics and treatment centers dealing with trauma-based therapies, what is the standard of care?”

The use of hypnosis in medical practice has long been controversial. Through hypnosis, the therapist helps patients learn to control their own state of awareness, which can help them change their own actions and thoughts from negative ones to positive ones. However, because patients are in a state of deep relaxation they are highly receptive to ideas, images, and suggestions, thereby making patients vulnerable to suggestive ideas or memories, as in the alleged case mentioned above.

PTSD and dissociation are relatively common in patients seeking treatment for their eating disorders. In general, trauma is a significant stress which can precipitate psychiatric symptoms, including the core symptoms of an eating disorder. Most 24 hour treatment centers – including McCallum Place –focus on resolving the dissociation, managing PTSD symptoms with medication, and building resources prior to any work aimed at uncovering new memories. When the eating disorder is severe, most patients do not have the internal resources to effectively cope with overwhelming situations. When a patient is malnourished, they are vulnerable and cognitive functioning can be compromised; anorexia is associated with trouble seeing the “big picture”, a process important for putting information in context. It is also important to recognize the possibility of inadvertently introducing false memories and take precautions against this potentially devastating outcome. In sum, whenever possible, active uncovering work is deferred until the eating disorder is more stable. When a patient has intrusive memories, dissociation, flashbacks, or avoidance related to a previous trauma, the therapist will focus on resolving symptoms, building self compassion and establishing safety. Supported exposures may be necessary to target avoidance, which is a barrier to recovery. The goal is always to reduce suffering by challenging cognitive distortions surrounding the event.

Why do so many people struggle with weight, body image and eating disorders?

Our world has changed faster than our bodies have been able to adapt. Our brains and body cues naturally encourage us to eat high fat, easy access, salty and sweet foods as if the supply will run out. But for most of us, it doesn’t. Artificial food additives trick our senses and may contribute to addictive feeding behavior. Other biologic and environmental factors make some individuals especially prone to overeat. Blaming the individual does not address the complexity of the problem.

In fact, Stigma can contribute to shaming, teasing and isolation of those who struggle with obesity.

This can have grave consequences in children leading to dangerous dieting, low self-esteem, depression and eating disorders. Those struggling with weight gain need support. It’s important to fight hate speech and confront shaming behavior.

What does BMI have to do with health?

Although high BMI is associated with significant health risks, not everyone with a high BMI is unhealthy.

In fact, some may be healthier. It is possible to be at a healthy weight at a variety of sizes. Extremes in weight, both too high and too low, are associated with increased health risk but it is eating and exercise behavior that are the most important.

Many athletes have high BMIs due to increased lean muscle mass. Helping individuals focus on finding and accepting a natural body weight range that will allow them to remain active and healthy is much more important than focusing on BMI or weight norms or beauty ideals. Review of growth curves and hormone levels can help physicians set appropriate weight range goals.

What is the relationship between binge eating and weight gain?

Lots of people find comfort in food, but binge eaters feel a total loss all control over what they are eating. Binge eating is so associated with guilt and shame that most people binge in secret. Parents may discover that food is missing or find food and wrappers hidden in the house. Most of the time, family members underestimate the problem. Binge eaters are typically very restrictive in their diets in between binges. Dieting is a significant trigger of binge eating in adolescents.

Over eating and poor eating habits are associated with an increased risk of weight gain which, over time, may lead to obesity. Even so, it’s important to remember that not all overweight kids and adults binge eat and few develop binge eating disorder. When binge eating disorder does occur, it tends to develop after obesity is already present. Childhood depression is also a risk factor for developing binge eating. Depressed children are more likely to have poor body image and negative self-concept, which may drive inappropriate dieting. Dieting is a major factor associated with the onset of binge eating in adolescents. Kids who are distressed about their weight should also be screened for vomiting after eating.

Anorexia Ruling Could Impact Coverage

A U.S. appeals court has ruled that an anorexic woman's insurance company must pay for her treatment at a residential facility because of a California law that requires insurers to provide the same coverage for mental illness as they do for physical illness.

The ruling may have important implications for how insurance companies pay for treatment for patients with mental illnesses in states with strong mental health parity laws.

The case involves Jeanene Harlick, a 37-year-old woman from California who has suffered from anorexia for more than 20 years. In 2006, she began intensive outpatient treatment, but her doctors said she needed inpatient treatment, according to the written decision from the U.S. 9th Circuit Court of Appeals in San Francisco.

Topshop has pulled images of a waif thin model from its online advertising following concerns that it could encourage eating disorders.

The 18-year-old model Codie Young appeared on Topshop’s latest online lookbook modelling a range of fashions and accessories.

A spokesperson from eating disorder charity Beat says: “We wouldn’t comment on an individual but we want to see diversity in images from the fashion industry to reflect the modern world and people’s different shapes an sizes. Images such as these can be a trigger for people suffering with eating disorders.”

The model, who is a UK size 8, denies suffering anorexia and defended the images on her personal blog.

A Topshop spokesperson says in a statement: “Topshop is confident that Codie is a healthy young woman and we do not feel it necessary to remove her from our imagery based on your feature. However we do recognise regretfully that the angle this image has been shot at may accentuate Codie’s proportions making her head look bigger and neck longer in proportion to her body.

“Topshop is proud of it’s heritage of celebrating individual-looking girls who offer an alternative more unusual beauty, however we take your comments very seriously. As you may have noticed we have taken down that specific image at the earliest opportunity.”

It has replaced the images in question with different shots of the model.

Standing in her Upper East Side condo, 28-year-old Karen is hoisting up a baby boy, revealing biceps that could give Michelle Obama a run for her money. The glamorous blonde whips out her homemade vegan apple walnut puree — fine for her 1-year-old, she says — but too indulgent for her. Karen went from an extra-small to a medium during her pregnancy, gaining 43 pounds, which left the former legal powerhouse feeling like, well, a house.
“I was obsessed with the fact that I didn’t become super-skinny right away,” says Karen, who asked that her real name not be used. “Every day, I strolled with my son and saw skinny moms around me — women who looked perfect, and I put crazy expectations on myself.”

“Beckham’s in her eighth month now, and looking too thin continues to be part of [her] plan — healthy or not,” says Lisa Cohn, a registered dietitian in Manhattan. “She hasn’t been looking as vibrant — pale face, dull eyes and more drained. If [Beckham’s] extreme post-baby weight-loss plan of shedding 20 pounds in a short period is true, it sends a dangerous message to other women out there.”
Indeed, the A-list trend of gaining a scant amount of weight during pregnancy has led to a similar psychosis among pregnant alpha women in NYC. Call them the mommyrexics — a breed of new moms who are pressuring themselves to bounce back to fighting weight days after they’ve left the hospital. Squeezing into maternity Spanx after having a baby isn’t good enough. These moms want to starve and jog themselves skinny if it kills them.

“I’ve seen a massive uptick in my clients feeling stress if they’re not down to their prebaby weight,” says Rosie Pope, the “pregnancy concierge” and star of Bravo’s hit show “Pregnant in Heels.”
Pope, who owns her own maternity boutique on the Upper East Side, even had to add an extra-small size to her range this year, due to popular demand.
“I’m worried about this trend, because even I couldn’t fit into it, and I’m small!” says Pope, who has a 4-month-old son.
The worlds of fitness and maternity are now intertwined for figure-obsessed moms-to-be. Runningskirts.com is just one of several lines — including maternityworkoutclothes.net and maternityactivewear.com — boasting about its new “maternity collections,” which meet the needs of pregnant women who exercise, sometimes excessively.

So who’s most likely to fall victim to mommyrexia?

“Women whose beauty is part of their allure, women who are frightened about losing their husband’s attention. You do see women — and it’s a vicious cycle — who are worried about losing their husbands, and will ignore the baby and even come to resent the baby,” says Isaac Herschkopf, a Manhattan psychiatrist who treats celebs with eating disorders. Herschkopf says he has seen mommyrexia trickle down from Hollywood to the masses, especially in wealthier communities.

While the normal amount of weight a woman gains during pregnancy depends on her body type, Herschkopf stresses that more than other factors, when it comes to taking it all off, “You want to lose it consistently.”
Nonsense, say the mommyrexics. While they struggle to avoid gaining much weight during pregnancy, after the baby is born, they want to whittle back down to their fighting weight — within weeks. “I’ve heard of women who have opted to skip breastfeeding altogether so they can keep their day schedules open immediately for spin class, hot yoga and five-mile jogs through the park,” says 27-year-old Bryce Gruber, founder of the lifestyle Web site The Luxury Spot.

“I actually knew of a mom that was skipping meals and drinking a ton of diet sodas for energy without calories after her baby was born,” says Gruber.
Meanwhile, at fitness centers, the increasing number of programs purporting to help new moms stay slim reflect the growing trend.

Joshua Margolis is a certified personal trainer at Mind Over Matter Health & Fitness, a personal training service in Manhattan that specializes in mommy-focused classes — with plenty of MILF- and Stroller Boot Camp options.

“Hell hath no fury like a post-partum woman trying to get back into the groove,” Margolis admits.
“We try to ride that wave and contribute to it for as long as possible.”

NEDA’s CEO spoke by teleconference on two occasions to Eric Galler, Vice President of Marketing, and Jeff Hagen, Director of Consumer Services, for General Mills. We are pleased to announce today that the company agreed to remove the ad within the next several weeks. As they have been buying time nationally and in regional markets, they will do their best to bring these ads to a close as quickly as possible. The HCG Diet. You've heard of it by now, we're sure, and so have we. We ran across this article about the new fad HCG Diet and here's what we like about this article:
1) The article openly speaks to this risk and reality: "You can't meet your nutrient needs on 500 calories a day. It's inducing a state of malnutrition," says Nancy Krupin, a Registered Dietician for Kadlec Regional Medical Center, and a certified Diabetes Counselor."
2) The article notes lack of research around the effectiveness of HCG.
3) The article also notes HCG has not been approved by the FDA for weight loss purposes.

Do you think these three points alone are worthy of reconsidering the HCG Diet? What do you think it means about our culture that many people subject their body to that which will induce malnutrition and use a product not approved by the FDA?

We can't help but think of our friend, Darryl Roberts, and his film America the Beautiful. In this documentary, he seeks to honestly expose our culture's answers to these questions and more, generally coming to this conclusion: We are a culture obsessed with beauty and tend to use whatever means to achieve this standard of beauty, even if it is unhealthy or toxic.

"A new review led by researchers of University of Michigan found eating disorder such as anorexia nervosa and bulimia are on the rise in U.S. children and adolescents. David S. Rosen, MD, MPH, of the University of Michigan Health System reviewed about 200 studies and estimated that 0.5 percent of adolescent girls suffer anorexia nervosa and 1 to 2 percent have bulimia. Eating disorders are particularly on the rise in boys who account for up to 10 percent of all cases of eating disorders." Read this article
